Wraparound Facilitator:

Peel Children's Centre is looking for a highly skilled, experienced and enthusiastic Wraparound Facilitator for the Wraparound program to provide services to children, youth and families.

Peel Children's Centre is an accredited children’s mental health centre that provides a continuum of high quality mental health services for children, youth and their families who are experiencing or may experience serious emotional difficulties. Our services are provided in a dynamic, client-centered environment that values flexibility, accessibility and responsiveness.

Our Peel Wraparound Process is a community-based outreach service that helps families with significant and complex mental health difficulties. Clients choose formal and informal support systems to "wrap around" their family. This process is based on our belief that every person, family, and community has strengths that can be used to make a real difference in the lives of children and families.
Annual salary range - $45,121 to $54,825 (5 step range)
Job Responsibilities
  • Guided by the community values of the Wraparound Process, you will assist families in a highly diverse ethno-cultural community in developing a Wraparound Service Plan.
  • Based on the family’s values, beliefs, culture, and coping strategies, you will help to identify and assess the child/youth/family needs and strengths and help them to articulate their goals and provide the necessary information to enable the youth/family develop a plan to support change.
  • Develop, implement and evaluate wraparound plans according to individual and family/caregiver needs;
  • Assist families in identifying potential team members that can support their goals, serve as the facilitator of these child and family teams, advocate for families in the program, provide linkages with community resources, coordinate access to services and/or treatment and monitor alignment of the plan’s implementation to youth/family goals.
  • Develop strong working relationships with the children, youth and/or their families and be responsible for planning and community mobilization efforts, which support and sustain a community-based, family-driven, strength-based and culturally and linguistically competent system of care for children and youth with serious mental health needs and their families.
  • Ensure that safety issues, impending crises and/or difficult transitions are identified and dealt with, by either, providing the necessary support yourself or by gathering a multidisciplinary, cross-sectoral team together, to address the issue and complexities of the child/youth and family.
  • Effectively assess, manage, design, and implement a crisis plan to ensure safety within the home/school/community setting;
Skills and Qualifications
  • Bachelor’s degree or 3 Year Diploma in Social Work or Social Services or a related field with eligibility for membership in the appropriate professional College or Association.
  • Have a minimum of three years’ experience working with children/youth and families in a mental health setting and/or developmental services sector;
  • Demonstrated knowledge and commitment to the Wraparound Community values; demonstrated cultural competence and ability to work within a child/youth/family centered, family-systems, strength based approach.
  • Previous training in Wraparound Process and Model is a decided asset;
  • Current knowledge of the legislation for children’s services;
  • Demonstrated family and youth engagement skills, facilitation skills and the ability to build relationships of trust and possess exceptional interpersonal and conflict resolution skills;
  • Demonstrated thorough understanding of systems theory, experience working with its principles and engaging systems to meet goals of children, youth and families;
  • Demonstrated ability to work both independently and collaboratively as a member of a multi-disciplinary team;
  • Possess excellent interpersonal and communication skills, both oral and written. Proficiency in languages in addition to English is an asset;
  • Strong skills in client record documentation and experience with client care information systems and Microsoft Office Suite;
  • Have a valid Ontario Driver’s license (Full G Class) and use of a vehicle with a comfort in driving clients throughout the Peel region (surrounding areas)
  • A satisfactory vulnerable sector police records check and pre-employment medical screening is required prior to the commencement of employment.
  • Available to work flexible hours, including evenings, according to client needs.
